The Social Frailty-10 (SF-10) is a brief measure designed to identify older adults who may be experiencing social frailty or who may be at risk of progressing toward it. It is intended for use as a screening and risk-stratification tool to support early identification of social vulnerabilities that may affect health, well-being, and functioning.

Scoring reference
- Robust: ≤ 1.50
- Socially Pre-frail: > 1.50 – ≤ 3.00
- Socially Frail: > 3.00
- Focused follow-up: Any answer scoring 4 or more points prompts a focused follow-up — the pattern of answers matters, not only the mean score.
